Please note that this option is available to patients of Markham Integrative Medicine only, and is provided as a convenient option for patients who wish to reorder their supplements. Patients are under no obligation to purchase supplements from Markham Integrative Medicine.

If you are a patient or the parent/guardian of a patient, please follow the link below to complete your order request form. Once confirmed, your order can be shipped right to your door, Canada-wide, or picked up from the clinic.
